In order to know the role of pharmacists in home care, and how far this program has developed, so this research was done. The research began with book study, continued with preliminary study, the amount and type determination, design and disseminate question form, data analysis, and conclusion. Based on question form analyses result; home care service was less known in the hospital community, 75,07% inpatients were not familiar with home care. Coordination with nurses in preparation, delivery, storage, and administration (documentation on patient's drugs) were the only role of pharmacist. About 66,66% patients, 100% doctors, and 100% nurses for home care service still needed of additional drug's information from pharmacists. 100% patients, doctors, home care nurses were lack of drug's information. Most of the lack information included: the way of dropping drugs (89,74%); drug's interaction (92,30%); and adverse effects of drugs (84,61%). No pharmacists involved in this program, has made the source of drug's informant were doctors (66,66%); nurses (25,64%); and his/ her family (15,38%). Patient's hope of source of drug's information were doctors (41,02%); pharmacists (33,33%); and nurses (17,95%). Based on data analyses, home care's programme was not socialized well in the hospital community. The existence of pharmacist was not helped the home care's program optimally. On the drug's information that delivered to the patients, there were still lack informations, include the way dropping drugs; drug's interactions; and adverse effects of drugs. text |
