In order to determine performance of insulation transformer, there is a condition assessment method that can be used. Condition assessment can be separated into two main parts. The first part is risk assessment. In risk assessment, relation of transformer insulation parameters which is involved to failure are determined. And then, the transformer insulation parameters can be sorted which one that has most influence to create failure. It will have the highest weighting factor than the others. The second part of condition assessment is monitoring diagnosis. Monitoring diagnosis is based on measurement of insulation transformer parameters to get the value and the quality of insulation. The transformer insulation parameters are water content, neutralization number, inter facial tension, breakdown voltage, colour, CO and CO2 content, and furan. After known all parameter values, each parameter is classified based on boundary value and condition level. The result of risk assessment and monitoring diagnosis are combined to determine transformer insulation condition. This method is applied for Kembangan transformer. And the result is Kembangan transformer insulation condition is running to failure after first failure in September 2007. After reclamation in april 2008, Kembangan transformer insulation condition is good. text
