Summary: | Main landing gear of Boeing 737-400 PK-GZN aircraft failed during taxi in at Syamsudin Noor Airport, Banjarmasin on July 23rd, 2008. The accident occurred because of main landing gear brake flange number one failure. The main landing gear failed after it has been operating for nine years and seven months and eight days (installed since December 15th, 1998) and 15.218 cycles since new. The
landing gear had never been overhauled. Overhaul will be held at the tenth years or 21.000th cycles. Failure analysis is performed to reveal the cause of this accident. The analysis which is carried out is visual analysis, fractography, chemical composition analysis, hardness test, stress analysis, and analysis of damage tolerance. Based on this analysis, it is found that the cause of its failure is
fatigue. This fatigue failure is known from the beach marks on fracture surface. The fatigue failure is originated from corrosion pits on five o'clock position of the brake flange hole (made of alloy steel AISI 4340). |
