Summary:WiSE-8 aircraft consists of several systems mutually connected. One of them is the flight control system (FCS). If failure occurs on the system it causes an accident with <br /> <br /> <br /> <br /> catastrophic or hazardous. Therefore an aircraft safety assessment of FCS failure is required to determine the aircraft safety level to identify which factors contribute. FCS safety analysis is never been done before. <br /> <br /> <br /> <br /> In this research, safety assessment process is conducted by using Functional Hazard Assessment (FHA) and System Safety Assessment (SSA) following SAE ARP 4761. Preliminary System Safety Assessment (PSSA) is not carried out in this assessment process because the detail design of WiSE-8 aircraft flight control system has been defined. <br /> <br /> <br /> <br /> Simulation of WiSE-8 is used for develop and verifying failure effect as the FHA’s output. The FHA become an input for SSA. The result of research shows, three the most critical failure mode is jamming/lock of elevator (catastrophic, 10-9), elevator floating (catastrophic, 10-9), and elevator surface broken (catastrophic, 10-9). The assessment shows the result of Common Mode Analysis <br /> <br /> <br /> <br /> (CMA) shows that the failure of the elevator and aileron-rudder in the Fault Tree Analysis have comply to the requirements. The Zonal Safety Analysis (ZSA) indicate that <br /> <br /> <br /> <br /> installation of FCS components complies the safety requirements. The SSA safety assessment still using hypothetical data, therefore to obtain more accurate results, it is recommended to use WiSE-8 flight test data.
