Hypertension treatment in Indonesia is expected to be simpler in the future with a program in national health coverage system, Prolanis. Anti hypertension drugs like thiazide diuretic is widely used in eventhough not as much as other class of anti hypertension drugs. The use of thiazide diuretics for hypertension in long term may impaired glucose tolerance which is caused by its potassium depletion mechanism. This research is an observational and prospective study to measure systolic blood pressure (SBP), diastolic blood pressure (SBP) and also random blood glucose (RBG) twice from April to June 2017 in Prolanis patients at Puskesmas Puter Bandung with antihypertension drugs regiment combination of hydrochlorothiazide and amlodipine and monotherapy of amlodipine 5 mg. The aim of this study is to determine the ability to maintain the blood pressure and the effect to impaired random blood glucose level between aforementioned treatments. SBP (p=0,2013) and DBP (p=0,418) show no significant difference between both regiments. Clinically, on the selection of combination of HCT and amlodipine SBP (123,00 ± 6,75 mmHg vs 122,00 ± 6,32 mmHg) and DBP (81,00 ± 3,16 mmHg vs 81,00 ± 3,16 mmHg) could be maintained for one month therapy, and so with SBP (117,50 ± 6,22 mmHg vs 119,17 ± 6,69 mmHg) and DBP (78,33 ± 7,18 mmHg vs 80,00 ± 4,26 mmHg) of monotherapy group. RBG level test between both regiment shows significant difference (p=0,043). Our findings show that the use of combination of HCT and amlodipine therapy may impaired RBG level, but can maintain BP as good as amlodipine monotherapy. This result may be used to reevaluate hypertension management in Prolanis setting. |
