Each hospital strives to provide good and quality medical services for patients to achieve hospital goals. However, many internal and external factors can affect a hospital in providing the best service. One influencing factor is risk. Almost every medical action saves potential risks. Hospital-based services laden with sophisticated technology and the complexity of diagnostic and therapeutic procedures make it possible for the risks to injure patients. Based on statistical data, 44,000 of 98,000 deaths occurring in Indonesian hospitals each year is caused by the risk of medical errors involving human and system factors. <br />

Therefore, in this final project proposed risk assessment activities related to information technology in hospital by using Failure Mode and Effect Analysis (FMEA) standard ISO 31010 and ISO 31000. FMEA is a method used to identify, assess, and analyzing a failure as well as the result to avoid the failure that has been identified. From the result of risk assessment at the hospital, there were 22 potential failure modes (7 potential failure modes are at low level, 9 potential failure modes are medium level, and 6 potential failure modes are in high level). In this final project, also provided tools for risk assessment that can be applied to RSUD Dr. Hadrianus Sinaga created using Microsoft Excel Visual Basic for Application (VBA) and Macro. |
