Summary:In clause 4.5 of ISO/IEC 17025, it outlines the requirements that will be met to ensure the competence of the subcontractor to complete any required test(s) that may impact the quality of results. To evaluate subcontractor competence is usually conducted by assessing the readiness of equipment and manpower involved in production activities. The process of assessing the laboratory by considering the readiness of equipment and manpower of laboratory has been done. However, other factors that significantly affect laboratory performance are to analyze the risk of failure that may occur in every production process in the laboratory, from the process of receiving samples to reports test results sent to the customer. In this study, we apply the quality function deployment (QFD) and failure mode and effect analysis (FMEA) methods for subcontractor selection in ISO/IEC 17025 standard. Our proposed method provides the score of a readiness of equipment and manpower (RoEM) and the risk priority number (RPN) of laboratory for selecting laboratory subcontractor process.
