Tandon employ over 100 worker which are placed in their workshop in Bantargebang, Bekasi. The products of PT. Tandon are water tanks, water treatment plants, pipes, and other steel based products activities that are dangerous. Accidents are one of the company problems in PT. Tandon Citra Murti area. To minimise accidents, researcher along with PT. Tandon Citra Murti implement risk management with the help of ISO 31000 to give us a framework on how to perform a risk management, Hazard Identification and Risk Assesment and Control (HIRAC) creates list of hazard that exist in PT. Tandon Citra Murti workshop, Fault Tree Analysis (FTA) provides list of causes, which includes immediate cause, intermediate cause, and basic cause of the hazard. Determining the solution to the hazard are based on the hierarchy of control which consist of eliminating the hazard, engineering, administration, and the use of Personal Protective Equipment (PPE). <br />

There are 127 hazards identified which 25 of them are clasified as high risk. Those 25 high risk hazards are then grouped into 10 hazards that have to be fixed or eliminated. There are 95 solutions or corective actions that came out of this research which consist of engginering control, administrative control, and signage. One of the problem that are discovered by using HIRAC is electrocuted worker, where by using FTA we could identify one of the causes to be broken cable which can be classified as unsafe condition, and from that we can conclude several solution which one of them is an engineering based solution ground fault circuit interuptor (GFCI). Beside installing GFCI there is an administrative solution which is safe operating procedure, where to eliminate a problem you have to eliminate all the causes. The 95 hazards iedntified could be classified into 4 groups following the swiss cheese model, where 21 hazards are caused by unsafe acts, 32 are caused by unsafe conditions, 26 are caused by bad supervision, 16 are caused by organitational influence. <br />
