Summary:Online questionnaires is widely used to obtain feedback from respondents in many kind of cases. Along with increasing use of questionnaires to meet various needs, online questionnaires require new features such as condition features based on the entries given by respondents and based on respondents' data in another system and the ease of changing questionnaires without having to re-program the questionnaire. In this Final Project, a website application is built that is able to generate and process questionnaires from metadata and this application can be integrated with an agency's information system. <br /> <br /> <br /> <br /> The need for an online questionnaire to regulate the emergence of questions with certain inputs or respondents with specific data, can be done using conditions based on respondents and conditions based on respondents' entries. The ease of changing online questionnaires without involving re-programming on the questionnaire can be achieved by using metadata. If the questionnaire on the application is made by compiling the metadata from the questionnaire, then by changing the questionnaire metadata the questionnaire will also change automatically. <br /> <br /> <br /> <br /> Application testing is done to ensure the application meets the needs described above. Based on the results of the test, the application is indeed able to meet these needs, but for a good interface to create complex conditions has not been reached. <br /> <br /> <br /> <br /> The conclusion obtained in this final project is that it has successfully created an online questionnaire application that is integrated in the agency information system. Applications can still be further developed in the answer expectation section and visualization of respondents' fields. <br />
