The determination of the dangerous events to the water supply chain is used the method of AHP. the use of questionnaire which consist of the pairwise comparison and weighting factor was done for the data collection. The respondent consist of academics and practition from PDAM, BPPSPAM and Dit. PAM, Ministry of Public Works, with the total of the respondent is about 41 persons. The hazardous events to the water supply chain is analysed based on the point of view of the quality, quantity, continuity, and affordability of water supply. After all the data is collected, then to be analysed with the AHP software Expert Choice11. The result shows that the priority in water supply severity is quality, followed by quantity, continuity and affordability. From this combination method (AHP-FMEA) obtained valid and consistent risk assessment by doing comparation risk assessment of PDAM Bandarmasih and PDAM Kota Malang.<p align="justify"> text
