Smartphone is one of the digital information technology devices that has been widely used to help the lives of the elderly. Through smartphones, the elderly can access various applications such as assisted medical technology and electronic health that can facilitate the elderly in medical treatment and supervision. However, the number of elderly smartphone users in Indonesia is still very small, namely 23.92% of the total elderly population. <br /> <br /> This study aims to determine what factors influence smartphone acceptance among Indonesian elderly. The study was conducted on 172 respondents aged 50 years and over who use smartphones. This study uses a reference model from Chen and Chan (2014) and Ma et al. (2015). The test is done by the partial least square structural equation modeling (PLS-SEM) method using SmartPLS 3 software. <br /> <br /> Based on the results, there are 3 factors that affect smartphone usage behavior directly, namely attitude towards using, self efficacy, and anxiety. In addition, there are 3 factors that indirectly affect smartphone usage behavior, namely facilitating conditions, cost tolerance, and health conditions. Age, gender, and education also proved to have an influence on smartphone usage behavior. <br /> <br /> Referring to the results of hypothesis testing, suggestions given to the stakeholders are the design focuses on uses that make life easier for the elderly, provide economical subscription packages, create training and socialization specifically for the elderly, promote active aging campaigns, and design low-threat smartphones or applications.<p align="justify"> <br /> text
