Summary:<p align ="justify">PT. X reported that 52 accidents were happened during 2012-2014. This research was conducted in grinding, stamping, machining auto and drawing and electroplating units. The objects of this research conducted the potential hazard identification, risk analysis, corrective and preventive recommendation. This research used Fault Tree Analysis (FTA) to systematically identify root cause of accident, Failure Mode Effects Analysis (FMEA) to completed FTA by identifying potential hazard and risk analysis based on Risk Priority Number (RPN) and Systematic Human Error Reduction and Prediction Approach (SHERPA) to identify potential hazard of human error based on hierarchial task analysis. Results of FTA indicated that the root cause of five major categories of occupational accidents in PT X 2012-2014 generally was lack of management control fot occupational health and safety. The results of FMEA indicated highest RPN in grinding unit was 252, stamping unit was 336, machining auto unit was 800 and drawing and electroplating unit was 300. The results of SHERPA indicated potential hazard of human error in grinding unit were action errors and checking errors; potential hazard of human error in stamping unit were action errors and checking errors; potential hazard of human error in machining auto unit were action errors and checking errors and potential hazard of human error in drawing and electroplating unit were action errors and retreival errors. The results of FTA, FMEA and SHERPA indicated potential hazard of PT X were gram intruded to eyes, inhaling metal dusts, inhaling dromus mists, noise exposured, ergonomic, crushed by stamping machine, cutting tool injury and inhaling chemical vapor. Corrective and preventive recommendations for PT X were subsitution, engineering controls, administrative controls and personal protective equipment.<p align ="justify">
