One amongst theories regarding accident causation was that most accidents triggered by unsafe or at-risk behavior, and control of this behavior became one of the keys to accident prevention. The occurrence of an accident contributed to the increasing operating costs of an organization through increased production costs and reduced efficiency. In addition to the high accident rate, it would not be easy for the company to earn the trust of the employer. Therefore, the company should have a role to prevent accidents. Accident prevention activities may be classified into two, namely retrospective (after an accident occurred) and prospective (before an accident occur). This research evaluates safety in PT. X through both activities. Incident analysis as a retrospective, and safety observation card as a prospective activity. Incident analysis conducted with root cause analysis (RCA). Safety observation card analysis conducted with statistics to review what observation reported. Both analysis is then reviewed to see whether there are any correlation between. The most frequently observation reported were Procedures and Housekeeping, Personal Protection Equipment, and Other. Analysis RCA with Matrix 4x4 and hazard category listed on safety observation card, whereas the root cause of the incident included in category Drops, Tools/Equipment, and Others (procedure). Therefore it is required to enhance worker’s observation capabilities in order to observe other potential hazards, through continuous socialization and training. text |
