As a means of supporting the country’s economy, this means of sea transportation is not be free from the fire incident that can happen on board. In this paper, the fire scenario on board the ship and in harbour will be identified using the Event Tree Analysis (ETA) method and the combined methods of fuzzy Fault Tree Analysis (FTA). Then, the control measures which needed to prevent and mitigate the fire scenario is determined. Fuzzy FTA is the method that can overcome the limitations experienced by conventional Fault Tree Analysis (FTA) which needed the exact probability values of the Top Event (TE). The fire incident studied consist of the fire accident in ship’s bunkering system and the fire accident on board the ship of Batu Mandi and Portlink I. Every way of fire scenario of event tree will be defined by fault tree diagram.Consequently the probability value of every TE will be determined. There are six fire scenario in the system of ships bunkering, and are six fire scenario on board ship. Generally, the fire causes on board ship are the decreasing of ventilation’s power in machinery space, and the bad habbit smoking activity in machinery and accomodation space, and the causes of the failure of fire protection system are the failure of passenger assisting and crow controlling and the fire drill conducted unperiodically and ineffective. Generally, the fire causes in ship’s bunkering system are the using of non-explosion proof tools and the bad habbit of smoking activities, and the causes of the failure of fire protection systems are the failure of notification measure of fire quick response team of fire city departement and the failure of availability of fire personal protections. The analysing of fire severity using Dow’s Fire and Explosion Index (FE&I) on storage tank is also been conducted that result in light category of fire impact with the result of liabilites economically accounted. Eventually, the control measure of fire hazards is determined considering the significant causes of failure based on the analysis of ETA and FTA and the hazard level of Dow's FE & I. text
