One of performance items in Badak LNG KPI is the number of Fire Incident in Plant Area. Badak LNG business process itself is categorized as a very high risk process, in which flammable hydrocarbon gas is processed in high operating pressures and cryogenic temperature. Therefore, a fire incident occurrence in the plant areas is highly undesirable in every employee’s point of view. Since 2016, Badak LNG had not achieved a target number of fire incidents occurrence at maximum 2 cases per year. Therefore this paper is written to look and evaluate solutions to prevent fire incident occurrence. Based on the Company record, Fire incidents which occurred in 2016-2018 could be categorized based on the precondition: operational, mechanical, electrical or instrumentational. Current Reality Tree method is utilized to find the general root causes for fire incidents. Based on the CRT method, there are 7 general root causes which contributed to fire incident occurrence: 1) Aging equipment; 2) Inadequate Communication; 3) Lack of Manpower Ability; 4) Low Nitrogen Plant Performance; 5) Inadequate Procedure; 6) Inadequate Preventive Maintenance’s Scope; 7) Budget reduction Solutions are developed to tackle all of root causes. In a plant-wise, retrofit nitrogen generation package plant is initiated to increase its performance. Other solution is to conduct fuse replacement program to minimize fire incident reoccurrence. In a process-wise, a reassessment of Preventive Maintenance’s scope should be conducted to response with its inadequacy and the issue of aging equipment. Employee development program such as leadership and communication skills training is to be intensified. In a people-wise, knowledge sharing culture is encouraged to minimize the gap of inadequate communication, and lack of manpower ability. Analysis is conducted to determine the priority of solutions using criterions such as completion time, complexity and cost. Based on the analysis, it is recommended to prioritize reassessment of Preventive Maintenance’s scope Task as the top priority solution. In the end, implementing all those solutions will increase the Company’s chance to avoid fire incident reoccurrence in Plant Area. text |
