Summary:The growth of the development and improvement of infrastructure in the public sector and for the private sector itself is currently cannot separated from the role of the import process. In 2016, Indonesia was ranked 30th out of 145 countries in the world as the largest importer country. One of the actors in the shipping industry to and / or from abroad is a forwarder. The performance of container transport vessel companies (KPPK) is one of the factors influencing the performance assessment of forwarder companies in the eyes of service tenants, namely importers. This study helps forwarder companies to have performance expectations for KPPK companies. It is expected that forwarder companies can better understand the risks that will be borne when they will / have collaborated with KPPK companies and can plan mitigation measures. To support this goal, this study proposes a model for evaluating the performance of KPPK companies, and then realized it using an evaluation form. Confirmatory Factor Analysis method is used to test the validity and reliability of the recapitulation results of 120 respondents' data obtained through an online questionnaire for 22 question indicators from 4 constructs, namely basic service, support service, cost, and customer service. The KPPK companies’ performance evaluation form can be used by forwarding companies as recording tools. Each construct and indicator have different rating weights according to the relative important index value.
