Nowadays, there is a significant growth of the use of herbs for therapy. One herb that shown antihyperglicemic effects is M. charantia fruit’s extract. The purpose of this research was to evaluate pharmacokinetics and pharmacodynamics interaction of metformin and M. charantia fruit’s extract when given concominantly. Studies were performed in six different groups of insulin resistant model wistar male rats, which were anesthethized by urethane (1.25 g/kg BW). The first group was given metformin (100 mg/kg BW, n=3). The second group was given commercial product of M. charantia fruit’s extract (341 mg/kg BW, n=3). The third group was given water extract of M. charantia fruit in water contained 1% of sodium CMC (341 mg/kg BW, n=5). The fourth group was given single dose of water extract of M. charantia fruit in solution dosage form (341 mg/kg BW, n=5). The fifth group was given water extract of M. charantia fruit and metformin concominantly and the sixth group was served as a control. Metformin concentrations in plasma were determined by HPLC and its pharmacokinetic parameters were determined. Plasma glucose concentrations were measured by GOD-PAP method. Administration of water extract of M. charantia fruit given in water contained 1% of sodium CMC and solution showed a glucose percentage decrease by 61.04% at 8 h and 44.51% at 3 h, respectively. In contrast, the commercial product showed insignificant decrease of glucose with percentage decrease only 15.07%. Concomitant use of metformin and water extract of M. charantia fruit showed a significant decrease of glucose by 83.47% at 3 h, which led to hypoglycemic shock and caused mortality. Therefore, pharmacokinetic parameters of metformin could not be detetmined. The decreased the dose of water extract of M. charantia fruit into 300, 200, dan 100 mg/kg BW showed the same effects when administered with metformin. The current study showed that water extract of M. charantia fruit and metformin could not be used concomitantly. text
