Summary:Conditon that decrease of kidney function which plays a role in the body's excretion system was called kidney failure. Madeira vine and sow thistle were traditionally used as treatments for kidney failure. This aim of this study was to determine the best effectiveness ethanol extract of madeira vine leaves, sow thistle leaves, and their combinations in the treatment of kidney failure and activity the combination of the two extracts to be synergistic, additive, or antagonistic. The test group consisted of madeira vine 50 mg/kgBW, madeira vine 100 mg/kgBW, sow thistle 50 mg/kgBW, sow thistle 100 mg/kgBW, combination of 50-50 mg/kgBW, and combination of 100- 100 mg/kgBW. Each group was compared against positive and negative controls. Induction of renal failure was performed with gentamicin 100 mg/kg BW/day and pyroxicam 3.6 mg/kg BW/day. Therapy for kidney failure is carried out for 4 weeks. Evaluation was carried out at the end of each week by measuring creatinine serum, urea serum and urine volume. Kidney index, TBARS, and catalase activity were determined In the end week of therapy. Histology of kidney was carried out at 1st and 2nd week of therapy. Data was performed statistically using t-student and one-way ANOVA with a confidence of 0.05. Analysis statistic on the 3rd and 4th weeks of theraphy showed significant differences between all the test groups with positive controls (p<0.05). The result of statistic urea and creatinine serum between combinations of 50-50 mg/kgBW with sow thistle 100 mg/kgBW and madeira vine 100 mg/kgBW did not show significant differences (p>0.05). Urinary volume from a combination of 50-50 mg/kgBW approached negative control compared to a combination 100-100 mg/kgBW and sow thistle 100 mg/kgBW. The lowest kidney index value is madeira vine 100 mg/kgBW and not significantly different from the combination 50-50 mg/kgBW (p>0.05). The results of TBARS and catalase activity from a combination of 50-50 mg/kgBW approached the negative control. Histology showed improvement in kidney function in several test groups compared to positive controls. The combination of 50-50 mg/kgBW has the best effectiveness in treat renal failure with additive activity.
