However, based on historical data from 2013 to 2017, fluctuating accidents occur per year with an average of 2,449 ± 231. Since the number of accident are not decreasing in time, it is necessary to prevent continuously. The Six Sigma method provides opportunity to conduct continuous prevention of accident. One of the Six Sigma method approach is five cycles of Define, Measure, Analyze, Improve, and Control (DMAIC). In the define phase, the Critical to Quality (CTQ) was determined, namely, the frequency and severity rates. In the measure phase, the safety process capability for the frequency and severity rates were 5.4 sigma and 4.3 sigma respectively. In the analyze phase, the priority of accident was analyzed and the root causes should further be determined, which turn out to be managerial functions. In the improve phase, layered recommendations were made, namely engineering, risk assessment, and managerial functions improvements. In the control phase, it was proposed to improve documentation, internal audits, use of control chart for monitoring accidents, and to use the Quality Control Circle (QCC) method for continuous improvement. text |
