Technically development of design, analysis and equipment has developed very rapidly, this is based on previous experience but there are deficiencies in the development of geotechnical management systems, document control, communication, competency development, inspection, testing and control of geotechnical designs. One of the causes of instability can be caused by a lack of quality support systems. For example, rock support procedures may be not appropriate to manage risks over time, improper installation practices, or the amount of damaged rock support materials and equipment that can be tolerated, etc. The instability in underground mining can cause safety problems and can result in economic losses (T. Szwedzicki, 2003). Based on research data, the results obtained from the use of resin bolts from year 2015 to 2018 are as many as 4079 resin bolts which are contributors resin bolt failed are 304 resin bolts or 6% equivalent. This failure can be caused by several factors so that needs to be evaluated. Six Sigma is an improvement strategy to eliminate dissipation, reduce costs due to poor quality and improve the effectiveness of operations so that it can meet the needs and expectations (Anthony dan Banueles, 2001). The six sigma methodology follows the cycle of DMAIC (Define, Measure Analyse, Improvement, and Control) with the use of Six Sigma, it is expected that quality problems can be resolved. text |
