Antidiabetic drugs is widely used biguanide and sulfonylurea, either in single or combination dosage form. In Garuda Primary Health Care, commonly uses metformin, glibenclamide and glimepiride. A study aimed to observe the effectiveness of therapy in patients and counseling planning. This study was cross-sectional observational studies retrospectively and concurrently during November 2015-April 2017. The subjects of the study were management of chronic ¸ð?± ?± ???ã? ?? ?ð±????(Prolanis). Data were obtained based on medical records and questionnaire distributed to the patients. Data were analyzed based on the achievement of therapy goals in each patient and the mean decrease of fasting plasma glucose (FPG) and postprandial plasma glucose (PPG) from antidiabetic which is used. The result showed that antidiabetic drugs in single or combination effectively decrease FPG (89.19%) and PPG (42.43%) levels in the first measurement after administration the drug. The combination of metforminglimepiride reduced the mean FPG amount 28.97 mg /dL and metformin-glibenclamide reduced the mean PPG amount 59.62 mg/dL. A total of 62.5% of patients did not reach the FPG and PPG target. Factors that affect effectivity of therapy is non-adherence of taking antidiabetic drugs. Counseling requirement were analyzed based on questionnaires to patients and interview with pharmacist. The results showed counseling should be conducted based on patient and information priority. Design of counseling materials covers disease counseling, lifestyle modification, physical activity, smoking, drug counseling, hypoglycemic treatment and strategy to improve adherence. text
