Summary:Diabetes mellitus lead to complications if not treated properly. Chronic Disease Management Program (Prolanis) is a health service system and proactive approach, which done to maintain the health of National Health Insurance participants with chronic disease, and achieve optimal quality of life with effective and efficient cost. The study was aimed to observe the effectivity antidiabetic therapy, the comparison of Prolanis implementation of controlled blood glucose, and analysis factors that affect visiting Medika Antapani health care frequency. Antidiabetic therapy effectiveness and the study about Prolanis and controlled blood glucose was done by comparing the postprandial blood glucose with normal range and the result was shown in percentage. Instead, analysis about visiting health care frequency was done concurrently using questionairre. The first study showed that therapy in 22,58% patient was effective while 77,42% patients less effective, and the percentage in diabetic category decrease from 51,61% to 32,26%.The result showed that non Prolanis group had 16,02% better control in the last examination. The last study showed that not checking blood glucose in another health care facilities had significant associations with visiting frequency in both Prolanis group (p = 0,025) and non Prolanis group (p=0,006). In conclusion, the antidiabetic therapy was less effective, the non Prolanis group has better control on blood glucose and factor that affect the high visting frequency was not examining their blood glucose in another facilities.
