Summary:To standardize therapy of disease, hospital established a clinical pathway which was compiled from various literatures and reflected the current clinical experience of health professional. In Cicendo Eye Hospital Bandung, there are applicable clinical pathways such as for primary open angle and angle closure glaucoma. This research was designed to analyze the level of ¸?®???’?®????ð ?®± ?? clinical pathway in hospital, assess the effectiveness of glaucoma therapy, evaluate clinical pathway to the current guidelines, and analyze the potency of drug related problems (DRPs). This research was a descriptive-observational study using retrospective design with purposive sampling technique from January –March 2016. Number of samples were calculated using Krejcie and Morgan formula. About 258 patients, 578 prescriptions, and 1023 visitations were included in this study. Data w±?± ?­? ð?±¸ »????±¸ð® ??±®??¸??? ? ??????± ?í± ¸?®???’?®????ð ?®±????í± effectiveness of therapy were set by intraocular pressure measurement, the clinical pathway was evaluated by comparing to the current guidelines, and potential DRPs were identified based on predetermined criteria. í± ?±?????» ¸?®???’?®????ð ?®± ??®?ð?ð® ?? ?í? ?? ?98%???? effectiveness of glaucoma therapy was 73%, algorithm and therapy target have not been included in current clinical pathway, and there were 40 cases of drug interaction, 4 cases of drug duplication, and 11 cases of inappropiate dose selection. Percentage of potential DRPs was 9,5% and it could be concluded that in 10 prescriptions have one potential DRP occured.
