Summary:Trihexyphenidyl is used as an anticholinergic drug used for treating side effect of extrapyramidal symptoms (EPS) which occured after using antipsychotics in schizophrenic patients. This study was aimed to investigate trihexyphenidyl usage patterns as well as the effects in schizophrenic inpatients at West Java Psychiatric Hospital. Of the 174 patients, only 88 patients met the criteria as study samples. Research was conducted on the patient medical records to find usage patterns of trihexyphenidyl, which was followed by observation using Modified Simpson-Angus Scale for patients who did not have record of extrapyramidal symptoms (EPS). Effects of trihexyphenidyl usage patterns were traced by assessing their association with the type of antipsychotic and the observed adverse effects. There were two usage patterns of trihexyphenidyl: in one case the drug was used after the onset of extrapyramidal symptoms (curative) which accounted for 38.6% of total uses, in the other case the drug was given before the onset of extrapyramidal (preventive) which contributed to 61.4% of total uses. The most common type of antipsychotic therapy with trihexyphenidyl was FGA-SGA combination. Percentage of FGA-SGA combination in patients who received curative trihexyphenidyl pattern was 35.29%, while that of the FGA-SGA combination in patients who receive preventive trihexyphenidyl was 40.74%. Trihexyphenidyl side effects found in patients with preventive pattern included urinary retention (9,26%), dry mouth (7,41%), constipation (3,70%), and blurred vision (1,85%). While those that were found in patients with curative pattern were dry mouth (5,88%) and blurred vision (2,94%). The results of this study indicate that the uses of trihexyphenidyl with preventive pattern tends to be accompanied by more severe side effects than curative pattern. This study further suggests that the class of antipsychotic used should be taken into account when using trihexyphenidyl. ii
