Kidney is an organ that has several functions including excretion, production of endocrine hormones, and metabolism. Renal impairment leads to drug related problems resulted from accumulation of drug, and other pathological disorders. Therefore, this study was aimed to identify those problems. A retrospective observational study was conducted to the inpatients of Hasan Sadikin General Hospital whom were treated during January to March 2015. Number of ? ???±?? ?¸±?±??ð?±¸ ­????ð?’?® ?®?? ?ð?????? ?¸ was selected using proportional sampling based on each diagnosis. Patients who used either one or more of studied drugs and drugs criteria were require dose adjusment, most recently used, and drugs used in comorbid treatment were studied. Medication use criteria was determined based on evedence-based literature, and used as a basis for Drug Related Pro­?±??’ ? ???ð?????Based on the study 54,59% of the total inpatient days was found to have inappropriate dosing, 604 drug-drug interactions, 21,61% were untreated indication from nursing day total of each patient, 31,99% were medication without indication, and 5 patient have disease progression. The result support that monitoring therapy must be done to decrease the number of drug related problems and to avoid disease progression.
