Formulary is a document that contains a set dosage of drugs chosen, the results of the latest clinical judgment of medical staff which has been approved to be used in Bumi Medika Ganesa (BMG) at a certain time span. Information of drugs in formulary is used as the basis for planning the procurement of medicines, so prescription services can be done maximally. Drugs in formulary should be updated regularly according to disease pattern, the development of treatment, and drugs on the market. Formulary in BMG has been compiled since 2007 which has never been revised until now. This study aims to determine the level of compliance to the formulary and draw up proposal of formulary revision. The study begins with a preliminary study to identify the level of compliance by comparing the consumptive data to the formulary drug on BMG, and the understanding level of physicians about the formulary by distributing questionnaires. Results of preliminary studies will be a basis for drawing up new proposals of formulary and formulary system. Result of the study shows that 94% physicians know about formulary in BMG, but from consumptive data shows that level of compliance in writing drug prescription to the formulary is 54.88%. The reason of physicians who do not adhere to the formulary because physicians prefer other trade names outside the formulary or the active substance is not listed in the formulary. There are three drugs which never been used for more than two years, namely Kenacort-A, Scanneuron, and Urdafalk. There is one drug which already not exists on the market, namely Dextromethorphan as single. 62% physicians give advice to do formulary revision and want to be participated in determination of drug which be added in new formulary. The display format of drug information in formulary consists of the name of the drug which is based on therapeutic class, dosage form, strength, trade name, manufacturer's name, price, dosage, and indication.
