FORMAL INSTITUTIONS IN DILEMMA OF HARM REDUCTION PROGRAM AMONG INJECTION DRUG USERS IN BANDUNG CITY

Department of Health in 2003 have included the Harm Reduction approach as part of efforts to combat HIV / AIDS, as well as the National AIDS Commission (NAC) which states in the National HIV/AIDS Strategy for 2003-2007 because the data is based HIV / AIDS cases reported indicates that the mode of transmission through sharing needles with increasingly evident as the most influential factor in the HIV epidemic in Indonesia. At the end of 2005, Harm Reduction program began at the level discussed in Bandung Health Office considering several activities related Harm Reduction has been run by several Non Governmental Organization (NGO) in the city since 2000 as mapping and outreach Injecting Drug Users (IDUs) including distribution of sterile syringes to injecting drug users. The method used in this research is institutional analysis and development, which focuses on how the characteristics of the environment, attributes of communities and rules in used as well as the interaction of the actor in action arena in the implementation of the Harm Reduction program in Bandung city. The results of this study indicate there are gaps in the implementation of the Harm Reduction program with the expected goal is the achievement of a condition where a decline in the prevalence of HIV / AIDS among IDUs in the city with some important records related to the formal institutions implementing the program as well as other issues. Barriers faced by the carrying capacity of the facility is not adequate, the anonymity of IDU, relay inadequate budgeting and not optimal pattern for IDUs programs aimed to recover from the effects of their addictions. To support the sustainability of the Harm Reduction program takes effort to overcome the obstacles faced, including the strengthening of health services with open characteristics, strengthening of health resources, boost the Recipient Institution Obligation Report (IPWL) and strengthening of formal institutions in the region downstream to upstream.
