In PT Minyak Indonesia, in the three-year period from 2015 to 2018 there was a failure to identify hazards as the most common cause category, which is 35% compared to the other four categories in the top five categories of causes of incidents. In terms of material losses, the incident caused by failure to identify the hazard caused an immediate loss of USD 27,050.18. This study presents the correlation of DMAIC methodology in Six Sigma and Kepner-Tregoe which comprising of the five phases: define, measure, analyze, improve and control. In addition to incident prevention, the quality of root cause must be accurate and based on facts In this study, the tools to identify, analyze and determine the root cause(s) by using Kepner-Tregoe and Why Tree analysis. Then the decision making for the improvement is determined by using Kepner-Tregoe Decision Matrix. According to above analysis, the root cause of incident is inadequate onsite Job Safety Analysis (JSA) library due to Person Managing Control of Work (PMCoW) lacked knowledge regarding hazard, less allocation time to develop quality onsite JSA, and the JSA was copied from the library and was not updated to hazard in current location. The improvement plan has been start implemented on May 2019 shows that the availability of JSA library and JSA workshop to field leadership have improved the quality of hazard analysis process. It is recommended to conduct regular JSA and PMCoW training, furthermore review the JSA library by cross functional team whenever there is a change to ensure the quality of JSA is maintained in order to reduce number of incident due to Failure to recognize hazard. Finally, the correlation of Lean six sigma and Kepner-Tregoe methodology provide a business strategy with a systematic and logical approach for problem solving, not only in the areas of quality, productivity, cycle time but also for reducing number of incidents in workplace. text |
