Type 2 DM can implicate into another disease, such as hypertension. A patient who suffer from DMT2 and hypertension, will consume at least two medication (polypharmacy). This phenomenon decreases patient’s adherence. There are several methods to improve patient’s adherence, yet this study will try to develop an alternative method which focus on the writing and the presentation of medication’s information for patient of DMT2 with hypertension. This research is a case-control which is done prospectively. This study conducted at four Primary Care in Bandung City with 60 Prolanis patients involved and divided into two groups, control group and intervention group. To measure patient’s adherence, Fasting Blood Glucose, Two-Hour Post Prandial Blood Glucose, and Morisky Medication Adherence Score-8 (MMAS-8) questionnaire was used as parameter. The result showed that there were improvement in patient’s adherence based on MMAS-8 score, that are in control group from 36,67% to 46,67% for patients who have high adherence, for patient who have medium adherence showed no differences, stayed at 43,33%, and from 20% to 10% for patients who have low adherence. While in the intervention group, from 30% to 50% for patients who have high adherence, from 43,33% to 40% for patients who have medium adherence, and from 26,67% to 10% for patients who have low adherence. The result is analysed by paired t-test and independent t-test. The result showed that there were significant differences (p=0,002 based on MMAS-8 score, p=0,001 based on Fasting Blood Glucose, and p=0,048 based on Two-Hour Post Prandial Blood Glucose), between both group before and after intervention. Therefore, medication’s information leaf can be used as an alternative method to improve patient’s adherence. text
