Summary:Diabetes mellitus is a metabolic disorder that is included in the top ten diseases that cause death. Diabetes treatment usually uses conventional drugs such as gliclazide which is the first choice in patients with type 2 diabetes mellitus who are intolerant of metformin. In addition to conventional medicine, herbal medicines have started to be in great demand and become one of the focuses of research to get an alternative treatment for type 2 diabetes mellitus. One of the herbs that have been widely used and studied is bitter melon. The objective of study was to determine the effect of bitter melon aquoeus extract on the pharmacokinetics and pharmacodynamics of glycazide when used simultaneously. The study was conducted on three groups of alloxan-induced diabetic rat models. Group 1 was given single dose of gliclazide (33 mg / Kg BW, n = 3), group 2 was given aqueous extracts of bitter melon fruit (341 mg / Kg BW, n = 4) and group 3 was given a combination of gliclazide and aquoeus extract of bitter melon (n = 4). The gliclazide pharmacokinetic profile was affected by bitter melon extract, where interactions occurred in the absorption phase. Blood glucose levels were measured using glucosameters. Decreased blood glucose levels following administration of glicazide, extract of bitter melon and combination of gliclazide and extract of bitter melon 6 hours after dosing were 74.73%; 82.977% and 86.457%, respectively. The results of this study indicated the interactions between glyclazide and bitter melon extract in the absorption phase of gliclazide and inthe effect of the blood glucose levels
