Research done by literature study and analyze the aviation accidents reports. The cause of aviation accidents in form of fault tree based on sequence cases logic and Swiss cheese model. The result of this thesis is frontline human error also influenced by organization policy. For example crew stress and pressure caused by duty time limitation. To reduce aviation accidents, continues improvement of system’s weakness is a must. |
