The products are CBU car and CKD parts to meet domestic or foreign demand (DNJ company branches in other countries). In the period from April 2019 to March 2020, PT. DNI received complaints from importers 6 times related to problems with parts were found at the importer. Parts that were delivered to the importer cannot be used and need replacement parts to keep maintain the production process at the importer can run smoothly. The purpose of writing this case study is to find the root causes of the problem and propose improvements. Supply Chain Export Department and packing vendor X together conduct a root cause analysis of each problem that arose. The method proposed in solving problems in packing vendors is SIX SIGMA DMAIC. DMAIC consists of 5 stages: Define, Measure, Analysis, Improve, and Control. At the Define stage is using the voice of customers and SIPOC diagrams to define each party's needs, especially customers. The second stage is Measure, where activity in this stage is collecting the data on the number of problems, types of problems, mapping processes, cost impacts, and time impacts. The third stage is to Analyze, where activity in this stage is conduct analyzing the root causes using a fishbone diagram from the 4M side (Manpower, Method, Measurement, Material, and Machine). The fourth stage is Improve, where activity in this stage is to determine the right improvement to the root causes of problems. In this paper, there are 2 types of improvements: existing improvements for each of the root causes of the problem and 3 proposal improvements to solve the root causes of the problem as a whole. The fifth or final stage is Control, where activity in this stage is all improvements controlling and written in the implementation plan regarding the improvement schedule. The implementation plan has been agreed upon between both parties Supply Chain Export Department and Packing Vendor X. text
