Based on the accident data, 65% of the total work accidents are caused by human factors (human error) and often occur in the Operations Directorate (Ministry of Mining and Production). This study aims to determine human reliability at the Mining and Production Department using the HEART method as a tool for evaluating work accidents at PT Semen Padang. The evaluation of work accidents is carried out by research stages, namely classifying the types of tasks or jobs into Generic Task Types (GTTs) and determining the Nominal Human Unreliability value, identifying conditions that cause errors or Error Producing Conditions (EPCs), determining the Assumption of the proportion of errors (Assessed Proportion of Effect or APOA), calculating the Assessed Effect obtained from the multiplication of the total effect, and the proportion of errors for each EPC, and calculating the HEP (Human Error Probability). The main finding in this study is that of the 4 work processes in the Mining Department, there are 3 processes that have a probability risk human error High, namely in the process drilling task 3.4 drilling at a certain depth, blasting task 3.2 making a series in each blast hole, and crushing 3.1 regulates the speed of the crusher. In the Production Department all processes have a probability to risk human error high. The Process raw milling has one task, namely 4.2 the reduction of the size of the raw material (grinding). The Process coal mill kiln has 4 tasks, namely 3.2 conducting trials and adjustments, 4.1 pouring the raw material mixture into the kiln (homogenization), 4.2 carrying out the preheating process, and task 5.4 moving the material to the next stage. The Process cement mill has 2 tasks, namely 3.3 separating material from metal (impurities) using a metal detector and 4.3 firing the results (fine particles) into the cement silo with purging water. The conclusion of this research is that there are 4 processes in the Mining Department are drilling, blasting, crushing, and conveying which consists of 91 tasks. Of the 4 processes in the Mines Department, 3 of these processes have a high probability of human error risk and 5% of tasks are classified as critical with an average HEP value of 0.57. In the Production Department there are 3 processes, namely raw milling, kiln coal mill, and cement mill in total which consists of 39 tasks. All processes in the Production Department have a high probability risk of human error and 8% are classified as critical with an average HEP value of 1.52. The implication of this research can be used by companies as a consideration for improving the work system in the form of increasing SOPs, providing training to workers, and also increasing work supervision. text |
