From administrative data, 35 puskesmas from 26 sub-districts in Sumedang Regency have implemented e- Puskesmas since 2019, however their use has not been optimal because they does not met the proper level of acceptance. This study aims to analyze the factors that have an important effect on the intention to use an electronic-based public health information system in Sumedang Regency. Adoption of public health information system acceptance was assessed based on Technology Acceptance Model (TAM) method by considering the perceived risk or risk factors when using the information system (perceived risk). The collected data will be processed and analyzed using the PLSSEM method. The results of data processing indicate that the perceived usefulness factor with a value of ? = 0.001 and ? = 3.363 has a significant influence on the intention to use an electronic-based public health information system. Health workers have the intention to use the latest technology, referred as e-Puskesmas application, due to the benefits obtained when using the application. The benefits obtained, among others, can reduce the workload of health workers, thereby it will increasing the efficiency and quality of health services, also the performance and productivity of health workers can be much improved.
