SAFE WORK PRACTICE IN ISOLATION OF HAZARDOUS ENERGY SAFEGUARD IMPROVEMENT THRU VERIFICATION AND VALIDATION PROGRAM

PT ABC is one of the big oil and gas company in Indonesia. PT ABC concentrate its business in upstream process. Over than half a century, it has produced more than 12 billion barrels of oil from onshore and offshore field area. PT ABC have an objective and commit to protect its people from Serious Injury and Fatality (SIF) at work site. One of the critical facility with complex job process is Drilling and Completion (D&C) Operation Area. Drilling and Completion (D&C) is a process by making a hole in the surface to the ground to obtain the source of oil inside the ground. Also it is responsible to make the well ready by conducting the maintenance and service program to maximize the well production Since most of the operational activity in PT ABC are categorized as a critical activity, and the potential SIF condition during work process is quite high then PT ABC established an initiative program named Verification and validation Program (V&V). V&V regularly come at site to observed and ensure all the critical safeguards are available and functioning. Therefore the observation result is reflecting into scores system. V&V have 2 kind of scoring systems: Conformance and Assurance Score One of the critical activity at site that is become focus item to be observed by V&V is Isolation of Hazardous Energy (IHE) Activity. IHE activity is a critical and important task which have to be completed in the beginning of job process during the maintenance and service process. Isolation of hazardous energy is required to protect workers from the releasing of the hazardous energy. These energy could endanger the workers and the potentially put the workers into the serious injury and fatality condition. According to the 6 months data in 2020 the IHE score is below expected target and the chart looks fluctuate with up and down in several months. There are caused by repetitive findings which press down the scores. Overconfidence and poor job planning and communication are the major error trap found at site. Therefore, this research aims to improve the observation score of IHE activity by developed systematical mitigation from the protocol procedure, human behavior practice, and supervision at site. These action will be monitored in 6 months and will be reviewed periodically. By the end of its monitoring timeline ,the result show a great number and increase the expecting target score, the project has success and recommended to be expanded to another job and facility.
