For ensuring an appropriate therapy was realized by prescription review, and for improving medicine compliance, by counseling. Therefore, the planning and preparation of drug counseling service and prescription review for outpatient had been carried out. The methods that were used, were preliminary study, determination of research location and sample number, designing and disseminating questionnaires, collecting and organizing data, analyzing data. The questionnaire results were: Patients experiences in drug using: inappropriate of drug using (41.43%), inappropriate of drug use schedule (70.99%), patients asked drug information to pharmacist (2.81%); Patients experience in drug counseling: counseling was unknown (90.28%), patients received drug counseling (20.72%), patients received drug counseling in Immanuel Hospital (56.80%), patient received drug counseling from physician (85.19%), patients received drug counseling from phamacist (6.17%); Patients need of counseling: patients needed counseling service (96.93%), counseling duration that was acceptable to 49.87% patients is about 5-10 minutes, one of counseling materials that was needed by patients was a drug side effect (57.54%), the most preferable counseling location for 65.73% patients was a special place, patients chose physician as a counselor (66.50 %), patients chose pharmacist as a counselor (13.81%), pharmacist was unrecognizable as a counselor (78.77%), need of written information (73.91%). Counseling service tools were designed, they were: counseling procedure, counseling form, counseling satisfaction form. The prescription review in satellit DC II found incompleteness of: patient name (1.54%), patient age or weight (7.81%), physician name (0.56%), drug quantity (0.33%), and signature (1.44%). It was found 1.43% pharmacokinetic interactions, 0.79% pharmacodinamic interactions, and 0.08% drug duplications. It could be concluded that counseling service is important to be applied in duration 5-10 minutes; the counseling materials were drug name, intended use and expected action, contarindication, dosage, common advers effect, advers drug reaction, direction of administration, drug use schedule, proper drug storage, drug interaction and drug-food interaction; The Pharmacists as a counselor should be socialized; prescription review regularly was needed. <br /> <br /> text
