Summary:In hauling and dumping activities on the mining operation, there are potential hazard that caused the risk of accidents. Haul truck are the primary means of haulage on surface mining that mostly used during operation process. According to the Mine Safety and Health Administration, there were 40 fatalities that caused by haul truck on surface mining between 2008 and 2021 at United State of America. Safety issues that related to haul truck are an important concern for mine management, authority policy, and researchers. This research use two method of analysis AHP (Analytical Hierarchy Process) and RCA (Root Cause Analysis), Analytical Hierarchy Process carried out for weighting the factor of accident due to haul truck accident at hailing and dumping activities on surface mining. Root Cause Analysis is used to determine the root cause of the accident which will be used as a benchmark in the recommendations for corrective action. Result of the study indicate, the biggest factor that caused haul truck accident is human factor with 35,64%, then management factor with percentage value 32,57%, then environment factor with percentage value 19,36% and equipment factor being the lowest factor with percentage value 12,43%. Human and management factor are the dominant factors causing a haul truck accident in hauling and dumping activities. The modified analytical hierarchy process method can be used to classify risk of haul truck accidents, by producing 5 classifications of risk, very low risk, low risk, medium risk, high risk, and very high risk. There is recommendation for corrective action that made from 6 cases analyzes based on root cause analysis as a form of follow-up that similar accident in the same place or other places do not repeated.
