Summary:In the previous study, the colonic release of coated-pellet contains ibuprofen has been developed. The purpose of this research is to determine anti-inflammatory effect of coatedpellet contains ibuprofen in local inflammatory model in colon (inflammatory bowel disease/IBD), compared to gastric-release ibuprofen and mesalamine as the gold standard therapy for IBD. Rats divided into 5 groups, which are normal group, positive control group, group with ibuprofen suspension 18 mg/kg bw, group with mesalamine 180 mg/kg bw, and group with coated-pellet contains ibuprofen 18 mg/kg bw orally. Rats were induced with 80 mg/kg bw TNBS through intrarectal route. Twenty four hours after induction, the preparations were given daily up to 14 days. Disease Activity Index was obeserved daily during the experiment. After 14 days observation, all rats were dissected and macroscopic observations were done. Observations were including colon’s weight, length, index, and damage, adhesivity, number of lession on small intestine, gastric’s damage, and tissue histology with hematoxylin-eosin (HE) staining. Based on Disease Activity Index (DAI), compared to positive control group, group received ibuprofen pellet showed more rapid recovery (after day 12), followed by mesalamine’s group (after day 13), and ibuprofen suspension after day 14. Since 8 th day, DAI of group received ibuprofen pellet had approached DAI of normal group, meanwhile DAI of group received mesalamine and ibuprofen suspension had not approached DAI of normal group until the end of experiment. Ibuprofen pellet also showed best ability to suppress the lesions in small intestine (10.00 ± 3.35) compared to mesalamine (11.00 ± 2.24) and ibuprofen suspension (12.33 ± 2.66). Ibuprofen pellet could suppress peptic ulcer (ulcer index = 2.69) compared to ibuprofen suspension (ulcer index = 6.13). Colon index of ibuprofen pellet (0.82 ± 0.07) also approached the normal group (0.67 ± 0,03). Furthermore, colon and gastric histology showed colon and gastric condition of ibuprofen pellet group were similar to normal group, followed by mesalamine and ibuprofen suspension. Coated-pellet contains ibuprofen showed better local anti-inflammatory effect in colon compared to mesalamine and ibuprofen suspension.
