Some factors affecting its ability to cause liver cancer are genotype and subgenotype, x gene mutation, and HBV DNA level. This research was aimed to determine the genotype and subgenotype of HBV isolates collected from patients based on its x gene nucleotide sequences and to detect its mutation, and to recommend a patient counseling based on the result. Method: Primer design, amplification and nucleotide sequencing of X gene, genotype and subgenotype determination and mutation detection were conducted in this research. The counseling was given based on genotype and subgenotype data obtained from BLAST analysis and HBV DNA level. Result: Molecular analysis showed that within analyzed isolates, two of them did not have any mutation, while three of them have one or more mutations. Nevertheless, the detected mutations are different from the mutations suspected to be the risk of hepatocellular carcinoma (HCC). The mutations found were C1480A, C1488G, A1565G, T1590G, C1464T, and C1675T. BLAST analysis showed that all patients were infected by HBV of B genotype, while four of them were infected by subgenotype B3 and one patient was infected by subgenotype B2. All of the samples were B genotype, which are not likely to cause severe liver diseases and hepatocellular carcinoma (HCC) compared to genotype C. But all patients need to be aware of the risk of HCC or cirrhosis development regarding high HBV DNA level. Patients were advised to monitor their disease status more often, including liver function test, HBV DNA level, HBeAg and anti HBeAg level concentration, and liver imaging. It is intended to monitor the disease progression so the doctors will be able to detect early signs of liver cancer. Patients are given general counseling for hepatitis B and specific counseling about the risk of liver cancer development which is caused by high level of HBV DNA. Conclusion: No detected mutations were correlated with increased risk of HCC, but based on relatively high HBV DNA titer, the patients need to be specifically counseled about its relevance with the risk of developing HCC. text
