Summary:Prolanis (Chronic Disease Management Program) is a government program in the health care system and a proactive approach that is implemented in an integrated manner involving participants, Health Facilities and BPJS Health. The purpose of Prolanis is to encourage participants with chronic diseases to achieve optimal quality of life with an indicator of 75% of registered participants visiting the First Level Health Facilities having good results on specific examinations of Type 2 DM and Hypertension according to the relevant Clinical Guidelines so as to prevent complications from the disease. (Kementrian Kesehatan RI. 2016). This study aims to describe the drug interactions of prolanis patients at UPT Puskesmas Ibrahim Adjie in the period October-December 2020. This study involved 81 prescription sheets. There are 12 prescription sheets that have the potential for drug interactions with a total of 19 findings. The type of interaction that occurs the most is interaction with a moderate degree of 73.68% and a major one of 26.32%. The type of drug interaction that occurs the most is the type of pharmacodynamic interaction with 52,63%. Then the pharmacokinetic interaction was 21,05%. Drug interaction between Lisinopril and Metformin is the most commonly found with 26,32%.
