Technological development has affected the way surveys are conducted. One of the most effective and efficient tools of performing data gathering for the purpose of survey is by using digital questionnaire. Data that are acquired from the answers of respondents through digital questionnaire can be used for certain purposes, such as gathering insights, help create innovation, improve the efficiency of business process, and receive feedbacks. The high amount of data volume and the large numbers of respondents really emphasizes the importance of having a reliable web-based online survey application. The web-based online survey application are made to facilitate survey makers to create questionnaire based on their needs. From the respondent’s perspective, this application could also facilitate the respondents in answering questions that are distributed by the survey makers effectively. The output of this bachelor’s thesis, which is in the form of web-based application, will be tested through user testing.
