The process in producing ammonia, urea and NPK involving high pressure, high temperature and hazardous chemical. PKT is having mega projects that are development of new ammonia, urea and methanol plant in West Papua, new soda ash plant and new NPK plant in Bontang, and plan to go initial public offering in 2022. These strategic plans need to be secured and escorted in term of safety and health aspect. Well execution of these plans will give growth to company and achieve sustainable competitive advantage. During the production process, incidents related with safety and health in PKT in past 6 years were still occurred. These past incidents and the risk of incident to re-occur are threats to company business sustainability. DMAIC tool is selected and used to provide Plan Do Check Action (PDCA) cycle to improve business current performance. Define step is to understand the process business in PKT with help of simple SIPOC analysis. Measure step with pareto tool is used to measure and prioritize existing issues. The risk of safety and health is further analysed through risk assessment which based on risk matrix. Root causes of past safety incidents that have been identified on investigation are classified into PSM elements based on OSHA standard. As result of pareto tool, process safety incident was selected as prioritized issue to be solved. Furthermore, analysis is made to find the root cause and plan the improvement. Improvement step is arranged to avoid the reoccurrence of past safety issues and to arrange strategies in closing the gaps. In this study, tool to set strategies and objectives and translate strategies and objectives into performance measurement is using the balanced score card. Control step to ensure competitiveness, continuous improvement, and the effectiveness of risk treatment is carried out through benchmarking and risk monitoring and re-assessment. Effectiveness of mitigation plan and current control measure on risk will bring risk after treatment into a safer state. Benchmarking is served to compare previous performance, current performance and targeting future performance that is more challenging and comparing company safety performance with global company as top safety performers. This is intended to set a challenging performance and give opportunity to PKT having perpetual improvement. text
