Positive cases of Covid-19 in Indonesia were first detected on March 2, 2020. The arrival of the vaccine brings fresh air to return conditions to normal. PT Bio Farma is the only SOE in Indonesia permitted to produce and distribute the Covid-19 vaccines. In distributing vaccines from Bio Farma to consumers, the cold chain is vital to maintain the quality of vaccines, ensuring effective use of products, and public confidence in vaccines. During the distribution of the Covid19 vaccine from early January to December 2021, several cases of temperature excursion have been officially reported to Bio Farma. 10 out of 7.800 shipments of the Covid-19 vaccine, or approximately 0.128%, have been reported to Bio Farma as experiencing temperature excursion. This research adopts qualitative methods. The qualitative method consists of two types of data (primary and secondary) covering direct observation at PT Bio Farma, interviews, historical study, and literature study. The business analysis is explained based on the DMAIC Six Sigma Framework, PESTLE Analysis, Resource-Based View (RBV), and Root Cause Analysis (RCA). Based on the study, the main causes of temperature excursions causes are high employee workload, Bio Farma has not followed the latest standards from WHO, and differences in perception between regions. Based on the interview and discussion results, there are three best alternative solutions to reduce the number of temperature excursions that change dry ice to PCM, implementing the Internet of Things (IoT) and implementing blockchain-based solutions. The feasibility study's result for PCM use is that the PCM procurement budget is lower than the risk of loss due to temperature excursions. IoT submissions consist of smart containers, smart vehicles, and Bio Farma SmartApp. The blockchain used to solve the distribution vaccine problem has a framework composed of six layers. This alternative solution is expected to help Bio Farma reduce the incidence of temperature excursions during the distribution of the Covid-19 vaccine. The business process that will be proposed is divided into three phases. Implementing alternative solutions will be carried out according to the PDCA method. text
