This research began with preliminary study, inpatient room sample decision, design and disseminate question form, prescription drug listing, data analysis, counseling tools design, and counseling procedure design. Based on question form analyses result, about 100percent patients never received counseling service. About 55.59percent patients at two inpatient room and 93.92percent nurses needed counseling service. The most preferable time for 72.62percent patients to receive counseling was between 9 to 12 in the morning. Counseling duration which acceptable to 32.96percent patients was about 15 minutes. Types of informations that patient needed were drug indication (26.04percent), administration instruction (21.90percent), side effects (21.89percent), dosage (19.58percent), miscellaneous (side effects handling, allergy, disease and its symptom, brand name of the drug) about 10.59percent. On the other hand, nurses needed information about drug administration (21.62percent), indication and contraindication (20.27percent), side and toxic effects (18.24percent), drug preparation (17.57percent), and drug interaction (16.22percent). The most effective method in counseling service was individual counseling, taken from the answers of 65.92percent patients and 81.76percent nurses. This research also carried out counseling tool kit containing: counseling procedure, counseling form, counseling service satisfaction form and prescripted drug list as a baseline to design drug information database. Based on question form analyses, counseling was needed by the patient. Informations that were needed by the patients and nurses could be the baseline in preparing drug information database. Pharmacists responsibility as a counselor were not well known by the patient and nurse. Thus, pharmacists should socialize and work actively on that service. Nurses might give drug counseling service after being educated and trained by the pharmacist. <br /> <br /> text
