DEVELOPING ACCIDENT PREVENTION PROGRAM PRIORITY BY KEPNER TREGOE MODEL BASED CASE STUDY OF PT PERTAMINA EP REGIONAL 2 ZONE 7

Pertamina EP Regional 2 Zone 7, a section of Pertamina EP, is a Upstream Oil and Gas Company operates in West Java that conduct high risk activities in exploration and production. According to the accident historical data, there are 40 work related accident cases occurs in Pertamina EP Regional 2 Zone 7 in the period of 2018 until 2021. In this period, the accident case is tend to raise. There are 9 accident case recorded in 2018 and 2019, meanwhile 11 accident case occurs in 2020 and 2021. During the same period the top five accident frequent sequentially are First Aid Incident (11 case), Property Damage (10 case), Nearmiss (7 Case), Medical Treatment Incident (6 case) and Environment Damage (4 case). Accident caused by multiple factors that require many recommendation and program to prevent the similar accident. The established accident prevention program in Pertamina Region 2 Zone7 is generate from all accident data in Pertamina working areas around Indonesia. As the result of these condition, the accident prevention program become out of focus and need to be the prioritized based on accident case data from Pertamina Region 2 Zone 7 working areas.The accident prevention program priority is determine by the combination of Kepner Tregoe Method, Pertamina DNV SCAT accident root cause and Pertamina Procedure. The combination method used to describe the root cause of accident case, identify the accident prevention program criteria, relevant alternatives accident prevention program based on the accident historical data, and choose the alternatives. The alternaltives solution is consist of the enhancement of established prevention program and proposed the new prevention program. Some of Established prevention program are still relevant with the historical data in Pertamina Region 2 Zone 7 such as Iman (Ingatkan Saya jika Tidak Aman) Campaign, Strengthen Work in Progress (WIP) Assesment in CSMS (Contractor Safety Management System), Job Safety Meeting before Start Working and BHL (Basic HSSE Learning) Training refreshment. Menwhile the new proposed program are Task Observation / Safety Awareness Supervision, Buddy System work for high risk job, Periodically Conducts random BHL (Basic HSSE Learning) test, and Mandatory Checklist Form for Supervisor. The implementation of the alternatives solution are embedded in OTP (Objectives Target Program) of HSSE (Health Safety Security and Environment) Pertamina Region 2 Zone 7. The OTP contains the program purposed, detail program, the output and the evidence of program, time schedule and progress monitoring of program. The implementation program are scheduled runs in 5-6 month started form July 2022.
