Summary:Sappan wood (Caesalpinia sappan L.), clove leaf (Syzygium aromaticum (L.) Merr. & L.M. Perry), and neem leaf (Azadirachta indica A. Juss.) have been traditionally used by native Indonesians as antibacterial agent and immune-system booster. This study aims to provide scientific evidence of the immunomodulatory effect of sappan wood ethanol extract (SWEE), clove leaf ethanol extract (CLEE), and neem leaf ethanol extract (NLEE) through carbon clearance method on Swiss Webster mice. A total of 36 mice were divided into nine groups, (1) negative control (CMC-Na 0.5%), (2) dexamethasone 2 mg/kg bw, (3) Stimuno® 30 mg/kg bw, (4) SWEE 100 mg/kg bw, (5) SWEE 200 mg/kg bw, (6) CLEE 100 mg/kg bw, (7) CLEE 200 mg/kg bw, (8) NLEE 100 mg/kg bw, and (9) NLEE 200 mg/kg bw. Treatments were given orally for seven consecutive days except dexamethasone which was injected intraperitoneally only on the seventh day. On the seventh day, all groups were injected with carbon suspension intravenously and blood samples were collected at 0 minute and 15 minutes after the injection. Blood sample absorbance was measured by microplate reader and then used to determine carbon clearance rate and phagocytic index. Liver and spleen were also taken to determine organ index. Both doses of SWEE, CLEE, and NLEE showed immunostimulant effect with phagocytic index of 1.68±0.21; 2.02±0.39; 1.62±0.36, 2.36±0.43; 1.88±0.35; and 2.49±0.56 respectively. SWEE 200 mg/kg bw, CLEE 200 mg/kg bw, and NLEE 200 mg/kg bw groups showed significant increase in liver organ index while CLEE 200 mg/kg bw, NLEE 100 and 200 mg/kg bw groups showed significant increase in spleen organ index.
