Summary:Prebiotic is a carbohydrate which is known to have an activity of repairing skin barrier function by stimulating growth of commensal bacteria. The impact of dermatitis on skin is dry skin, which is a result of skin barrier damage, that increases Trans-Epidermal Water Loss (TEWL). Moreover, skin inflammation is observed and can be measured with erythema index. The objective of this literature review is to determine the moisturizing activity with the parameter of TEWL, balancing of skin microflora, and suppression of skin inflammation activity with the parameter of erythema index of prebiotic on skin, also to design a topical prebiotic formula. From the inclusion and exclusion criteria in this study, 103 journals were selected and will be discussed in this study. This study evaluates the activity and formulas of prebiotics based on 5 different topical formulation. Topical dosage form used in those 5 formulas are cream, lotion, and gel. Prebiotics used in those 5 formulas also include: xylitol, and gluco-oligosaccharides. From these 5 formulas, it was concluded that prebiotics show moisturizing, anti-inflammatory activity, and suppressing growth of S. aureus. Then, a formula is designed based on one of the 5 formulas. The base formula is chosen according to the result of activity tests, in which the formula must give a significant effect (p<0.05) on decreasing S. aureus colony, erythema index, and TEWL. A lotion formula design is proposed which is composed of: prebiotic, glycerine, phenoxyethanol, ethylhexylglycerine, PEG 80 hydrogenated castor oil, disodium EDTA, and aquades.
