In 2018, Ciparay, Pacet dan Kertasari district was selected as part of the PAMSIMAS III program, divided into three years of processing (2018-20192020). These recipients' selection is based on the high number of Diarrhea patients in their districts and bad sanitation facilities. Therefore, the objective of this study is to conduct the Benefit-Cost Analysis to measure the cost-effectiveness of the program. This study uses Benefit-Cost Analysis by measuring the cost of building PAMSIMAS from the Budget Plan or Rencana Anggaran Biaya (RAB). The benefit will be conducted by how much a person's income will be lost if hospitalized using the Cost of Illness theory. The results of this study show that all of the designated villages (Ciparay, Pacet and Kertasari) shows the positive result in reducing Diarrhea by 49%, 29%, and 27% respectively from 2019 to 2020. The Benefit-Cost Ratio of the programs for all the designated district shows more than 1, which means that this program is beneficial to reducing Diarrhea. Referring to the results from the designated district, the Government should continue this program by creating a similar or replicating the current program in the future for another district in Indonesia to enhance the text
