Summary:PT Berau Coal is one of the largest coal producers in Indonesia which located at Berau Regency, East Kalimantan Province. To manage operational activity with 18.541 manpower safely, PT Berau Coal implemented Safe Work System in order to prevent accidents and incidents which consists of 5 layers of incidents prevention where each layer consists of various activity to help workers identify risks, manage and mitigate risks, prevent potential hazard from both personal and works factors, and as well prevent unsafe condition and action which are direct causes of accidents. Every activity in Safe Work System should be implemented and obeyed by every worker working in PT Berau Coal. In 2021 there were 149 cases of incidents and accidents whereas 2 of the cases are LTI case which include 1 fatality and 1 major injury case, from investigation report it is obtained that 91% of related causal of incidents and accidents in 2021 were due to gap in safety program implementation. At the same time, review of safety program implementation shows that only 3 out of 13 programs are scored below baseline whereas the other 10 programs scored 90-100% range. This causes concern that implementation of safety programs inside the Safe Work System doesn’t lead to targeted safety performance and success in safety management. Early Warning System is an initiative to harmonize leading indicator and lagging indicator performance through enrichment of leading indicator measurement and evaluation through the introduction of correction factor, which is obtained from reports of unsafe condition and action detected from inspection, audit, or compliance assessment report. Another advantage is gap can be detected earlier to done corrective intervention before it becomes a contributor to an incident. Early Warning System is a proactive effort to trigger continuous improvement.
