ABSTRACT: <br /> <br /> <br /> <br /> <br /> As the biggest information and communication (infocom)company in Indonesia, Telkom is supported by Infratel Division to manage all infrastructures needed in order to run all services and infocom access. Nowadays telecommunication industry grows so fast, inventions of new technology and market competition force any infocom company to rearrange all industry operation risk involved. Operation risk is need to achieved company objective and strengthens the company ability to survive. Therefore the importance to arrange a well established operation risk model is required for mapping all risk and plan scenarios to overcome many possible risks to come. To accomodate the Sarbane-Oxley Act (SOA) risk management system, every risk management that exist and still using the job description process will have a conversion to a business process system. The objective is to make easier for anaysis in every risk occured and how to handle it. From severity matrix analysis, comes one extreme risk, nine high risk, five medium risk, and two low category operational risk. Lot of attention focused on two top risk category, on the extreme and high category operational risk. Risk scenario will be divided by three based on time. The first one is risk prevention, the objective is to lower any possibility for the risk occured. The second is risk mitigation which is to overcome any risk that already coming. And the last is recovery, the action to recover all process after the risk have been overcomed.
